How many swimming pool illness outbreaks happen each year in the U.S.

U.S. health departments reported an average of 37.3 treated recreational water outbreaks and 569 outbreak cases a year from 2009 to 2023. The series is uneven: large Cryptosporidium years (2016) and the 2020 pandemic dip move it more than any trend, and the most recent years are still filling in.

Counts rise from 24 in 2009 to peaks of 58 in 2016 and 56 in 2018, fall to 9 in 2020 during the COVID-19 pandemic, then rise to 30 in 2021, 32 in 2022 and 50 in 2023.0153045602009: 2420092010: 342011: 3820112012: 362013: 3320132014: 412015: 4020152016: 582017: 4420172018: 562019: 3520192020: 92021: 3020212022: 322023: 502023

2020 was the first COVID-19 pandemic year; CDC links pandemic-era changes partly to diverted reporting resources. 2023 can still rise as late reports arrive.

In 2016, 58 U.S. outbreaks and 2,020 cases were reported, the highest case count since 2008; Cryptosporidium accounted for 41 of those outbreaks and 1,862 of the cases (CDC NORS).

In 2020, the U.S. file shows 9 outbreaks and 36 cases, 80.7% fewer outbreaks than the 2015 to 2019 annual mean (PC&S analysis of CDC NORS). CDC’s 2021 summary suggested that jurisdictions may have changed outbreak reporting during the pandemic because resources were diverted to the COVID-19 response (CDC WBDOSS 2021 summary).

From 2019 to 2023 the United States averaged 31.2 outbreaks and 304 cases a year, against 37.3 outbreaks and 569 cases a year across 2009 to 2023 (PC&S analysis of CDC NORS).

Cryptosporidium caused 186 U.S. outbreaks (33.2%) and 4,944 cases (57.9%) from 2009 to 2023 (PC&S analysis of CDC NORS). Counting only laboratory-confirmed Cryptosporidium outbreaks, the share of cases is 56.8%; excluding the 2016 peak year, it is 47.6% (PC&S analysis of CDC NORS).

Legionella caused 190 U.S. outbreaks (33.9%) but only 940 cases (11.0%) from 2009 to 2023, because most Legionella outbreaks are small; it accounted for 413 hospitalizations (59.9%) and all 22 deaths (PC&S analysis of CDC NORS).

CDC’s 2015 to 2019 review reached the same pattern: Cryptosporidium caused 76 outbreaks and 2,492 cases, and Legionella caused 65 outbreaks and all 13 deaths (CDC MMWR, 2021). For 2000 to 2014, CDC attributed 89% of cases in confirmed infectious outbreaks to Cryptosporidium (CDC MMWR, 2018). Outside the United States, a 2026 review of 23 peer-reviewed articles on pool outbreaks from 2020 to 2025 also found Cryptosporidium and Legionella the most frequently reported pathogens (Steinberg et al., 2026).

Chemical exposures caused 68 U.S. outbreaks and 1,293 cases (15.1%) from 2009 to 2023, 54 of them at pools (PC&S analysis of CDC NORS). CDC notes chemical outbreaks are likely under-reported because hazardous materials responders and infectious disease staff may not share reports (CDC MMWR, 2021). Emergency department data on pool chemical injuries are covered in PC&S pool injury statistics.

Pools, hot tubs and splash pads: which venues cause which outbreaks

Pools account for most outbreak cases; hot tubs account for most outbreak deaths. Pool outbreaks are mostly Cryptosporidium and chemical; hot tub outbreaks are almost entirely Legionella and Pseudomonas, and no hot tub only outbreak in 2009 to 2023 was caused by Cryptosporidium.

Hot tubs or spas alone were the venue in 176 U.S. outbreaks (31.4%) but only 916 cases (10.7%) from 2009 to 2023 (PC&S analysis of CDC NORS). Those hot tub outbreaks produced 310 hospitalizations and 20 of the 22 deaths (CDC NORS).

Legionella caused 147 of the 176 hot tub outbreaks (83.5%) and Pseudomonas caused 18 (10.2%), together 93.8% of hot tub outbreaks and 88.2% of hot tub outbreak cases (PC&S analysis of CDC NORS).

Splash pads or water playgrounds alone were the venue in 24 U.S. outbreaks and 279 cases from 2009 to 2023; Cryptosporidium caused 16 of them (CDC NORS). CDC’s splash pad review counted 60 outbreaks and 10,611 cases from 1997 to 2022, with Cryptosporidium in 40 outbreaks and 9,622 cases (CDC MMWR, December 2024). In June 2021, a Kansas wildlife park splash pad was linked to a Shigella outbreak with 21 cases among study respondents and a separate norovirus outbreak with six (CDC MMWR, 2022).

277 of 560 U.S. outbreaks (49.5%) from 2009 to 2023 began in June, July or August; July had the most, with 108 (PC&S analysis of CDC NORS).

158 of 190 U.S. outbreaks involving Cryptosporidium (83.2%) began in June, July or August from 2009 to 2023 (PC&S analysis of CDC NORS). Local swim season length by city is covered in PC&S pool season length by city.

March was a secondary peak with 56 U.S. outbreaks from 2009 to 2023 (CDC NORS). 62 of 176 hot tub only outbreaks (35.2%) began from December to March (PC&S analysis of CDC NORS). CDC reported the same bimodal pattern for 2000 to 2014, with 56% of outbreaks starting in June to August and 9% in March (CDC MMWR, 2018).

CDC’s report on a March 2023 Pseudomonas aeruginosa outbreak at a Maine hotel pool, where 23 of 35 responding guests (66%) developed ear pain, rash, or foot or hand pain, notes that hotel Pseudomonas outbreaks predominantly occur during January to April (CDC MMWR, 2024).

Where outbreaks are reported: treated recreational water outbreaks by state

Forty-four states, the District of Columbia and Puerto Rico reported at least one treated recreational water outbreak from 2009 to 2023. State totals mostly reflect how actively health departments investigate and report, so they rank surveillance as much as risk.

Florida reported 79 outbreaks from 2009 to 2023, 56 of them Legionella; Minnesota reported 64 and Ohio 51, with Cryptosporidium the most common cause in both (CDC NORS). Ohio’s 51 outbreaks produced 1,341 cases, the most of any state, including 638 from the July 2016 Cryptosporidium outbreak (CDC NORS).

Why CDC outbreak counts change after publication

NORS is a live system. Counts in CDC’s first published summaries are lower than the same years in the October 5, 2026 download of the file, by about 12% for 2015 to 2019 and 36% for 2021. All counts are also a small fraction of modeled illness.

CDC’s May 2021 MMWR counted 208 treated recreational water outbreaks and 3,646 cases for 2015 to 2019, using reports filed by February 4, 2021 (CDC MMWR, 2021). The current file lists 233 outbreaks and 4,053 cases for the same years, 12.0% and 11.2% more (PC&S analysis of CDC NORS).

CDC’s 2021 summary counted 22 outbreaks and 219 cases using data as of January 19, 2023 (CDC WBDOSS 2021 summary). The current file lists 30 outbreaks and 349 cases for 2021, 36.4% and 59.4% more (PC&S analysis of CDC NORS). The 2000 to 2014 count differs by only 0.8% (PC&S analysis of CDC NORS).

CDC modeled 5.61 million illnesses linked to recreational water in the United States in 2014, including 42,300 hospitalizations and 1,290 deaths (CDC, Emerging Infectious Diseases, 2023). That model covers lakes, rivers and oceans, sporadic illness and swimmer’s ear (79% of the estimate), so it is not directly comparable with outbreak counts; it shows that outbreak reports capture a small share of swimming-related illness. Across all water exposures, CDC estimates that 1 in 44 people in the United States gets sick from waterborne disease each year (CDC).

For comparison across water types, CDC counted 214 drinking water outbreaks reported from 2015 to 2020, most of them caused by Legionella (CDC MMWR, 2024).

Chlorine, hot tub water levels and the Model Aquatic Health Code

CDC publishes recommended disinfectant levels and a model code for public pools, hot tubs and splash pads. The code is guidance, not federal law, and applies only where a state or local jurisdiction adopts it.

  • Hot tubs: free chlorine at least 3 ppm, or bromine 4 to 8 ppm, with pH 7.0 to 7.8 and water no hotter than 104 F (CDC Healthy Swimming).
  • Diarrheal contamination response: MAHC hyperchlorination raises free chlorine to 20 ppm for 12.75 hours to inactivate Cryptosporidium (CDC MMWR, 2021).
  • 2024 MAHC, 5th edition (posted December 16, 2024): cyanuric acid at 300 ppm or higher requires immediate remediation, and free chlorine above 10 ppm with bathers present is an immediate-closure hazard (CDC MAHC).
  • CDC states that one in eight routine public pool inspections and one in seven routine public hot tub or spa inspections result in immediate closure (CDC MAHC).
  • CDC found improper disinfectant levels in about 20% of 13,864 routine public hot tub inspections in 16 jurisdictions in 2013 (CDC MMWR, 2018).
CDC suggests swimmers check chlorine or bromine and pH with test strips before using a pool or hot tub.

  10. Pools alone were the venue in 269 U.S. outbreaks (48.0%) and 4,862 cases (56.9%) from 2009 to 2023 (PC&S analysis of CDC NORS).
  11. Hot tub or spa outbreaks produced 33.8 hospitalizations per 100 reported cases in the United States from 2009 to 2023, against 3.5 per 100 for pool outbreaks (PC&S analysis of CDC NORS).
  12. Chemical exposures (chlorine, chlorine gas, chloramines and acids) caused 68 U.S. outbreaks and 1,293 cases, 15.1% of cases, from 2009 to 2023 (PC&S analysis of CDC NORS).
  13. Hotel, motel or resort settings accounted for 199 of 560 U.S. outbreaks (35.5%) from 2009 to 2023 (PC&S analysis of CDC NORS).
  14. Florida (79), Minnesota (64) and Ohio (51) reported the most U.S. treated recreational water outbreaks from 2009 to 2023 (CDC NORS).
  15. CDC estimates 5.61 million illnesses in the United States in 2014 were linked to all recreational water, about 9,590 for every case counted in that year’s treated-water outbreaks (CDC, Emerging Infectious Diseases, 2023; PC&S calculation).

Patterns across the registry

  • CDC’s public outbreak file lists 560 treated recreational water outbreaks with first illness from 2009 to 2023 in the United States, an average of 37.3 a year (PC&S analysis of CDC NORS).
  • Those 560 outbreaks caused 8,539 cases of illness, 689 hospitalizations and 22 deaths in the United States, 2009 to 2023 (CDC NORS).
  • Cryptosporidium, a chlorine-tolerant parasite, caused 57.9% of U.S. outbreak cases from 2009 to 2023, but only 30.5% from 2019 to 2023 (PC&S analysis of CDC NORS).
  • Legionella caused all 22 outbreak deaths and 59.9% of outbreak hospitalizations in the United States, 2009 to 2023 (PC&S analysis of CDC NORS).
  • Hot tubs or spas alone were the venue in 176 U.S. outbreaks from 2009 to 2023; Legionella or Pseudomonas caused 93.8% of them (PC&S analysis of CDC NORS).
  • 49.5% of U.S. treated recreational water outbreaks from 2009 to 2023 began in June, July or August; 83.2% of Cryptosporidium outbreaks did (PC&S analysis of CDC NORS).
  • Reported counts keep rising after first publication: the 2021 count grew from 22 outbreaks in CDC’s 2021 summary to 30 in the current file (CDC WBDOSS 2021 summary; CDC NORS).

Counts and rates we derived

Outbreaks and cases per year

How many treated recreational water outbreaks and cases does the U.S. record in a typical year?

FormulaOutbreaks (or cases) with onset in the period divided by years in the period

Inputs: fact rows #5, #6, #7, #337, #338

From 2009 to 2023 the United States averaged 37.3 outbreaks, 569 cases and 46 hospitalizations a year. For 2019 to 2023 the average was 31.2 outbreaks and 304 cases a year.

Limitations: Simple means; one year (2016) holds 2,020 of 8,539 cases, and the latest years are still filling in.

Cryptosporidium share of cases

How much of the outbreak illness burden comes from Cryptosporidium?

FormulaCases in Cryptosporidium outbreaks divided by all outbreak cases

Inputs: fact rows #347, #6, #392, #393, #394, #339, #338

57.9% of 2009 to 2023 cases (56.8% counting only laboratory-confirmed outbreaks; 47.6% excluding 2016; 30.5% for 2019 to 2023).

Limitations: Share swings with a few large outbreaks; recent years may still gain late Cryptosporidium reports.

Venue mix by outbreaks and cases

Do pools, hot tubs or splash pads account for more outbreaks and cases?

FormulaVenue outbreaks (cases) divided by all outbreaks (cases)

Inputs: fact rows #403, #404, #412, #413, #421, #422, #430, #431, #5, #6

Pools alone: 48.0% of outbreaks, 56.9% of cases. Hot tubs alone: 31.4% of outbreaks, 10.7% of cases. Splash pads alone: 4.3% and 3.3%. Multiple venue types: 12.7% and 25.3%.

Limitations: Venue as reported; outbreaks implicating several venues are kept separate rather than split.

Seasonality of onset

When in the year do outbreaks start?

FormulaOutbreaks with onset in the months named divided by all outbreaks in the group

Inputs: fact rows #540, #5, #541, #390, #542, #412

49.5% of outbreaks began June to August; 83.2% of Cryptosporidium outbreaks did. 35.2% of hot tub only outbreaks began December to March.

Limitations: Month of earliest illness onset, all years pooled; does not adjust for seasonal swimming volume.

Legionella and Pseudomonas in hot tubs

What share of hot tub outbreaks are caused by Legionella and Pseudomonas?

FormulaHot tub only outbreaks (cases) caused by each pathogen divided by all hot tub only outbreaks (cases)

Inputs: fact rows #416, #417, #412, #521, #522, #413

Legionella 83.5% and Pseudomonas 10.2% of 176 hot tub outbreaks, together 93.8% of outbreaks and 88.2% of cases.

Limitations: CDC data for 2000 to 2014 show Pseudomonas folliculitis outbreaks falling 22% a year, possibly because hot tubs are remediated without a formal investigation.

Severity by venue

Are hot tub outbreaks more severe than pool outbreaks?

FormulaHospitalizations divided by cases, times 100, by venue

Inputs: fact rows #414, #413, #405, #404, #415, #8

Hot tub outbreaks: 33.8 hospitalizations per 100 cases. Pool outbreaks: 3.5. Hot tub only outbreaks account for 90.9% of outbreak deaths.

Limitations: Severity among reported cases, not risk per user; people with Legionnaires disease often need hospital care, which raises the hot tub ratio.

Late-report drift

How much do outbreak counts grow after CDC first publishes them?

Formula(current file count divided by first published count) minus 1

Inputs: fact rows #333, #783, #310, #813, #331, #801

2015 to 2019: +12.0% outbreaks. 2021: +36.4%. 2000 to 2014: +0.8%.

Limitations: Part of the gap may reflect PC&S scope rules rather than late reports; the splash pad count runs 5.0% lower in the file than in CDC’s 2024 review.

Legionella overtakes Cryptosporidium

Has the leading outbreak cause changed?

FormulaCount of onset years 2017 to 2023 with more Legionella than Cryptosporidium outbreaks; change in mean annual Cryptosporidium outbreaks

Inputs: fact rows #340, #341 and yearly rows for 2017 to 2023

Legionella led in 7 of 7 years from 2017 to 2023. Annual Cryptosporidium outbreaks fell 86.3% from 2015 to 2019 to 2020 to 2023.

Limitations: CDC suggests part of the pandemic-era pattern might reflect reporting priorities and hot tub water quality not being maintained during low hotel occupancy.

Scale of unreported illness

How do outbreak counts compare with modeled recreational water illness?

FormulaCDC modeled recreational water illnesses in 2014 divided by treated recreational water outbreak cases in 2014

Inputs: fact rows #852, #262

About 9,590 modeled illnesses per reported outbreak case in 2014.

Limitations: Different scopes: the model includes untreated water, sporadic illness and swimmer’s ear. A scale indicator, not a detection rate.

Questions about the registry

How many swimming pool illness outbreaks were reported in 2023?

U.S. health departments reported 50 treated recreational water outbreaks with onset in 2023, with 379 cases, 60 hospitalizations and 3 deaths, in CDC’s public file updated December 20, 2024 (CDC NORS).

What causes most pool outbreak deaths?

Legionella. All 22 deaths in U.S. treated recreational water outbreaks from 2009 to 2023 occurred in Legionella outbreaks (CDC NORS), and CDC’s 2015 to 2019 review found all 13 deaths in Legionnaires disease outbreaks (CDC MMWR, 2021).

What is the largest pool outbreak in recent CDC data?

A Cryptosporidium hominis outbreak in Ohio with onset in July 2016 had 638 cases, the largest U.S. treated recreational water outbreak from 2009 to 2023 (CDC NORS; CDC MMWR, 2021).

Do hotel pools and hot tubs have more outbreaks?

Hotel, motel or resort settings accounted for 199 of 560 U.S. outbreaks (35.5%) from 2009 to 2023 (PC&S analysis of CDC NORS). CDC found 71 of 208 outbreaks (34%) at hotels or resorts for 2015 to 2019 (CDC MMWR, 2021).

Can splash pads make you sick?

Yes. CDC counted 60 splash pad outbreaks with 10,611 cases from 1997 to 2022, 40 of them confirmed to be caused, in part, by Cryptosporidium (CDC MMWR, December 2024).

Are pool chemicals a cause of outbreaks?

Yes. Chlorine, chlorine gas, chloramines and acids caused 68 U.S. treated recreational water outbreaks and 1,293 cases from 2009 to 2023 (PC&S analysis of CDC NORS).

How long should someone with Crypto stay out of the pool?

CDC advises staying out of the water until two weeks after diarrhea has completely stopped (CDC Healthy Swimming).

What chlorine level does CDC recommend for hot tubs?

At least 3 ppm free chlorine, or 4 to 8 ppm bromine, with pH 7.0 to 7.8 (CDC Healthy Swimming).

Why did pool outbreaks drop in 2020?

The U.S. file shows 9 outbreaks with onset in 2020, 80.7% below the 2015 to 2019 annual mean (PC&S analysis of CDC NORS); CDC’s 2021 summary suggested reporting changed as resources were diverted to the COVID-19 response (CDC WBDOSS 2021 summary).

Is CDC outbreak data complete?

No. Reporting is voluntary and outbreaks are likely underreported (CDC NORS). The 2021 count rose from 22 outbreaks in CDC’s first summary to 30 in the current file (CDC WBDOSS 2021 summary; CDC NORS).

Other questions

Can you get sick from a swimming pool?

Yes. U.S. health departments reported 560 outbreaks linked to pools, hot tubs and splash pads from 2009 to 2023, with 8,539 cases (CDC NORS). CDC says people get swimming-related illnesses by swallowing, having contact with, or breathing in mists of contaminated water (CDC Healthy Swimming), and that diarrhea is the most common cause of swimming-related outbreaks (CDC).

What is the most common pool illness?

In U.S. outbreak data, diarrheal illness from Cryptosporidium is the most common: it caused 57.9% of treated recreational water outbreak cases from 2009 to 2023 (PC&S analysis of CDC NORS). Across all recreational water, CDC’s 2014 model estimates swimmer’s ear (acute otitis externa) made up 79% of illnesses, but swimmer’s ear is rarely reported as an outbreak (CDC, Emerging Infectious Diseases). The Minnesota Department of Health also lists diarrhea as the most common type of recreational water illness (Minnesota Department of Health).

Does chlorine kill Crypto?

Not quickly. CDC reports Cryptosporidium can survive for more than 7 days in properly chlorinated pool water (CDC Healthy Swimming). CDC’s Model Aquatic Health Code response to a diarrheal incident raises free chlorine to 20 ppm for 12.75 hours, and UV or ozone systems can also inactivate the parasite (CDC MMWR, 2021).

How long after swimming do you get sick?

It depends on the germ. CDC says cryptosporidiosis symptoms generally begin 2 to 10 days after infection (average 7 days) (CDC); Legionnaires disease symptoms usually develop 2 to 14 days after exposure (CDC), and CDC tells anyone who used a hot tub in the last two weeks and develops symptoms to see a healthcare provider (CDC Healthy Swimming); hot tub rash usually appears a few days after sitting in a poorly maintained hot tub (CDC).

Are hot tubs more dangerous than pools for infections?

Hot tub outbreaks are smaller but more severe. From 2009 to 2023, hot tub only outbreaks averaged 5.2 cases against 18.1 for pool outbreaks, but produced 33.8 hospitalizations per 100 cases against 3.5, and 20 of 22 outbreak deaths (PC&S analysis of CDC NORS). No national exposure data exist to compare risk per use.

How the registry is compiled and checked

  1. Downloaded all 3,106 waterborne outbreak rows (primary mode = Water) from CDC’s public NORS dataset on data.cdc.gov (5xkq-dg7x) via the Socrata API on October 5, 2026; the file was last updated December 20, 2024.
  2. Kept rows whose water exposure is treated recreational water only (1,085 outbreaks, onset 1978 to 2023); 16 rows that combine treated recreational water with another water exposure were excluded, matching CDC practice of classing multi-source outbreaks separately.
  3. Coded venue from the NORS water type field (pool, hot tub or spa, splash pad or water playground, multiple venue types, other or unknown) and etiology group from the etiology field (scripts/classify.py).
  4. Tabulated outbreaks, cases, hospitalizations and deaths by year, etiology, venue, onset month and state, with analysis focused on 2009 to 2023, the electronic NORS era.
  5. Compared file counts with CDC’s published summaries (MMWR 2018, MMWR 2021, WBDOSS 2021, MMWR splash pad review 2024) to measure late-report drift.
  6. Added context from CDC Healthy Swimming pages, the Model Aquatic Health Code, CDC’s waterborne disease burden model and recent MMWR outbreak reports, each opened and checked against the claim.

Included: Outbreaks reported to CDC NORS with treated recreational water as the sole water exposure; CDC MMWR, CDC summary reports and CDC web pages; one peer-reviewed review for international context.

Excluded: Single cases (not outbreaks); outbreaks with mixed water exposures; untreated recreational water (lakes, rivers, oceans); cruise ships with foreign ports; news reports of outbreaks not in the CDC file; the July 2026 MMWR U.S. Virgin Islands hotel Legionella report (shower and sink water, not a pool or hot tub).

Conflicting numbers: Where CDC’s published summary and the current NORS file differ for the same years, both are shown (Table: CDC published outbreak counts versus the current NORS file). The file is used for trends because it covers every year consistently; published MMWR figures are quoted as published. The 2018 MMWR states Legionella outbreaks rose 14% a year in its summary box and 13% in its results; the results figure is used.

Derived metrics: All derived metrics are computed by scripts/03_build_tables.py and scripts/04_build_facts.py from the raw download and are listed in derived.csv with formulas and input rows.

Caveats

  • Outbreak reporting is voluntary and varies by state; counts understate illness and state totals reflect surveillance capacity.
  • NORS is dynamic: recent years (especially 2021 to 2023) are likely to rise as late reports are added.
  • No denominator (pool visits, hot tub uses) exists nationally, so these are counts, not risk rates.
  • The 2020 to 2022 figures are affected by the COVID-19 pandemic and diverted public health resources.
  • The public file has no facility names, cities or outbreak IDs and does not include onset-year 2024 or 2025 data as of October 5, 2026.
  • Pre-2009 data come from paper reporting with incomplete hospitalization and death fields.

Method last reviewed October 5, 2026.

Registry summary

U.S. health departments reported 560 illness outbreaks linked to swimming pools, hot tubs and splash pads with onset from 2009 to 2023, according to a PC&S analysis of CDC’s National Outbreak Reporting System public file. The outbreaks caused 8,539 cases, 689 hospitalizations and 22 deaths. Cryptosporidium, a parasite that can survive more than 7 days in properly chlorinated water, caused 57.9% of cases, and 83.2% of Cryptosporidium outbreaks began in June, July or August. Legionella caused all 22 deaths; 20 were in outbreaks linked to hot tubs alone, where Legionella or Pseudomonas caused 93.8% of outbreaks. The latest year in CDC’s file, 2023, had 50 outbreaks, the most since 2018, and Legionella has outnumbered Cryptosporidium every year since 2017. Counts are provisional: the 2021 total grew from 22 outbreaks in CDC’s first summary to 30 in the current file. Reporting is voluntary, so all figures understate illness.

Headlines the record supports

  • CDC data: 560 illness outbreaks tied to U.S. pools, hot tubs and splash pads in 15 years
  • Every death in U.S. pool and hot tub outbreaks from 2009 to 2023 involved Legionella
  • Chlorine-tolerant Crypto caused nearly 6 in 10 pool outbreak illnesses, CDC data show
  • Hot tub outbreaks are small but send a third of patients to hospital
  • Pool and hot tub outbreaks hit 50 in 2023, the most since 2018
